Outside haematopoietic stem cell transplantation, where engraftment is the entire point, many pre-clinical studies have found that when a stem cell repairs damaged tissue most of the benefit does not come from the cell taking up residence. It comes from what the cell releases: extracellular vesicles carrying microRNA, growth factors, cytokines and structural proteins. Collectively, this is the secretome.

This matters because live cell therapy is difficult. Following systemic administration, most transplanted cells never reach the target tissue – they are trapped in the small vessels of the lung and liver, those that do arrive carry risks of immune rejection. These types of products can also be at risk from uncontrolled growth, batch variability and handling complexity.
